Patient Referral Form

Please fill out the electronic form below if you are a referring clinician and would like to refer a new patient suffering from headaches, TMJ/jaw pain, neck pain, or vertigo to our Queensland TMJ & Headache Centre. If the patient provides consent we will keep you updated on your patients’ treatment and progress.

• Check availability for a patient online
• Book a patient an appointment online for headache, neck, or TMJ conditions
• Call the clinic for more information
• Enquire to request paper referral pads to be delivered.
